Terms and Conditions Agreement
This Application, Audition Video, and all supplemental materials submitted for consideration for the 2012 U.S. Army All-American Marching Band become the property of MENC: The National Association for Music Education (MENC). To protect students’ privacy, no personal student information from these forms will be released beyond MENC promotional efforts in conjunction with the program.
I, the Applicant, certify that the information submitted on and with this Application is complete and accurate to the best of my knowledge, and that all Terms and Conditions have been read and understood. I certify that I am an American citizen which makes me eligible for this national recognition. I certify that I have not been convicted of a crime as a juvenile or as an adult and I am in good standing in my community and at my academic institution. I certify that I am a member of my high school band and will remain a member through the U.S. Army All-American Marching Band performance in January 2012. MENC has permission to contact my school and/or Band Director if necessary to confirm the accuracy of this information and authenticate this application.
I understand that it is NOT acceptable to touch-up, overdub, edit, correct, or alter the audition video elements in any way to cover flaws. I warrant that no “doctoring” took place on any audio element and what is heard is the original recording of me.
I understand that all decisions of the judges are final and if selected, substitution or transfer of finalist status is not permitted unless sanctioned by MENC. I understand that if selected, I will be required to perform at the U.S. Army All-American Bowl, January 7, 2012 and to participate in all rehearsals scheduled from Tuesday, January 3 through Saturday, January 7, 2012. I understand that 2012 All-American Marching Band music or guard work will be provided in the fall of 2011 and that I must memorize all provided music or guard work before arriving in San Antonio for the 2012 All-American Bowl rehearsals. I understand that airfare from a United States city, hotel, and food will be provided to participate during this week. If for any reason I cannot rehearse and perform at that time due to injury or otherwise, I understand that I can be put on injured reserve, and MENC can select another applicant to perform in my place.
If selected, I agree to read the rules and regulations to be provided governing my participation and conduct, and I agree to comply with all rules and regulations governing my participation and conduct as a member of the 2012 U.S. Army All-American Marching Band.
I consent to the use of my name, likeness, address (city, state), academic information, school information, and reproduction of my sounds and performance in any and all media for the purpose of facilitating promotion, advertising, or otherwise in conjunction with this program, which may include announcements in local or national newspapers or other media including, but not limited to, television, radio, magazines, and the Internet for publicity, advertising and promotional purposes without compensation.
I understand that MENC, its officers, directors, agents, and employees shall not become liable or responsible in any way in conjunction with the services they provide, or for injury, death, damage, delay, or irregularity which may occur while I participate in the 2012 U.S. Army All-American Marching Band, unless caused by the gross negligence or willful misconduct of MENC. I also understand that I will be required to sign a medical release form giving consent for medical treatment if necessary and that all associated medical costs will be my responsibility upon my receipt of the medical care. If I do not follow program rules or staff direction, I understand that I will be responsible for any associated costs for damages and/or my early transportation home.
