• Name 
• Chapter ID number (if known)
• Name of your school
• Your phone number
• Your e-mail address
• An explanation of how the logo will be used
• What we can help you with
   

The ® (registered) should appear next to the Tri-M Music Honor Society logo or words "Tri-M" when they first appear on the page. Unauthorized copying or use of said trademarks is strictly prohibited. Tri-M logos are not available to be used on items for sale or profit.

Logos
